Once you have had your piggie for a while you will get to know their behavour and eating patterns etc and you would notice if something seemed different or if they were unwell. There are lots of sections on this forum that will help you with things to look out for.

When my Charlie was poorly he was less active, he didnt come out to get his morning veg and I noticed he was resting/sleeping a lot more than normal.

I agree with the above. It's a question of getting to know your pigs. Some things will be obvious, like a crackling sound when breathing, but sometimes it's just that your piggy is being quieter than usual, or off his food.
